FirstHx is a medical history intake software available for certain patients when they submit a consultation request on Maple. It is designed to provide additional context on the presenting issue and the patient’s medical history before the consultation.
How It Works
While the patient is waiting to be matched with a provider, they have the option to complete a series of medical questions.
This intake process typically takes 3-5 minutes to complete.
The collected information is shared with you to support a more informed and efficient consultation.
This integration helps streamline patient assessments, ensuring providers have key details upfront.
Viewing the Intake Form
During Triage
Click “View Intake” or the attached file to view the full intake form. You'll have an additional 30 seconds in the triage room to review the information.
During the Consultation
The completed intake form will be automatically added to your private notepad in the consultation room. To view the added notes, simply click Notepad.
After the Consultation
The intake information will be attached to the consultation summary as a PDF under "Uploaded files".
