FirstHx is a software integration that is built into select consultation request flows. It’s a medical history intake program that is designed to provide more information on the presenting issue and patient’s medical history.
When patients are in the Maple waiting room, they are given the opportunity to complete a series of medical questions which take on average 3-5 minutes to complete.

Click “View Intake” or the attached file to view the full intake form. You'll have an additional 30 seconds in the triage room to review the information.

The completed intake form will be automatically added to your private notepad in the consultation room. To view the added notes, simply click Notepad.

The intake information will be attached to the consultation as a PDF under Uploaded files.
