STI Notification and Reporting Policy

When a requisition ordered on Maple has positive STI results, there is a reporting requirement. Learn more in this article.

Overview

Patient and provider notification

  1. Positive STI results are received by Maple and posted to the requisition

  2. Maple automatically notifies the patient of any new results and that follow-up is required

  3. Maple manually informs the patient of the result and provides standardized follow-up recommendations

  4. Maple automatically notifies the provider of any new results using the requisitions tab

  5. Maple manually informs the provider of the result and any actions taken

Public Health form received

  1. Public Health form is received by Maple

  2. Maple attempts to contact the patient to collect outstanding information and fills in as much information as possible

  3. Maple sends the partially completed form to the provider

  4. Provider completes the form and returns it to Public Health

Provider responsibilities

As the ordering provider, responsibilities include the following:

  1. Ensuring that results have been managed appropriately

  2. Reviewing and completing Public Health forms associated with their requisitions

  3. Signing-off on completed Public Health forms associated with their requisitions

  4. Returning completed forms to Public Health

While Maple will attempt to provide as much pertinent information as possible, it is the ordering provider that will sign-off on the form and ensure that it has been completed to their satisfaction.

Patient notification

When a positive STI result is received for a Maple patient, Maple posts the result to the associated requisition. The patient will receive an automated email, prompting them to follow-up. Maple also reaches out directly to the patient to notify them of the positive result and advise follow-up with a General Practitioner within 48 hours.

Provider notification

Once Maple has contacted the patient, or has exhausted all reasonable attempts to reach the patient, Maple will notify the ordering provider. Maple will inform the provider of the positive STI result, the actions taken to inform the patient, as well as any messaging that was passed onto the patient. They will also prompt the provider to review the results and update prescriptions that were ordered, if necessary.

Public Health notification

Information collection from the patient

In the event that Maple receives a Public Health reporting form for a Maple patient with a positive STI, Maple will reach out to the patient to address any unanswered questions and collect the following information:

  1. Is the patient aware of the positive result?

  2. Has the patient followed up on the result?

  3. With whom and when did the patient follow-up?

  4. What, if any, treatment has the patient received?

Once the required information is collected, Maple will document all relevant information and fill in the associated sections of the Public Health reporting form.

Form completion and sign-off

Maple will fax the partially completed form to the ordering provider. To ensure timely and secure communication, Maple strongly recommends that all providers have access to a fax account.

The provider should review the form, add any other pertinent information, and sign the form. If no treatment has been provided, providers are encouraged to mention this on the reporting forms. Forms can be faxed back directly to Public Health once completed. Some Public Health departments may have a secure email option as well, please use caution with all PHI.
