The only information that is automatically populated from Maple Admin to lab requisitions is the patient’s full name and date of birth, as well as the ordering doctor’s information. The patient can print the form and manually fill out the other required information for the lab to accept.

As a reminder, for diagnostic imaging requisitions, only the information added to the Clinical Indication section will appear on the lab requisition (information entered in under “What is the clinical reason for this test” will be internal to Maple only).
