Step-by-step instructions for creating a lab requisition during and after a consultation.

Mobile app — Issuing a lab requisition in a consultation

  1. In the consultation, select the hamburger menu on the top right, then select Requisitions.

  2. Select the plus icon on the bottom right of the menu.

  3. Based on the type of test and province, select the requisition from the list provided. Complete any additional required fields.

Select a requisition to fill out

  1. In the consultation summary, click Requisitions.

  2. Select the plus icon on the bottom right of the menu.

  3. Based on the type of test and province, select the requisition from the list provided. Complete any additional required fields.

  2. Select the requisition required from the list provided, based on the type of test and province. Complete any additional required fields.

  3. The completed requisition can be viewed from the consultation summary, under the Requisitions tab.
