Lab requisition availability is based on the province the patient is currently located in, as well as the specialty and partnership plan. For example, diagnostic imaging requisitions are not available in Manitoba, Newfoundland, and Saskatchewan.

If you can’t find a requisition form with the test you would like, please use the primary care requisition form and add the specific test as a note under “Additional tests”.
