To refer a VirtualCareNS patient for in-person primary care following a Maple consultation, there are two steps:
Click the “Create diagnosis” option and select the location nearest to the patient from the dropdown.
Click the “Create requisition” option, select the (VirtualCareNS) In-person Referral Form, and include any relevant comments.
Once both a diagnosis and requisition have been submitted, the VCNS admin team will phone the patient directly to help arrange an in-person appointment for in-person care.
