Quebec has specific regulations on what content must appear in doctor’s notes in order for them to be accepted by workplaces and schools. They must include:
Date the certificate was issued
Patient's full name and address
Information necessary for the employer to perform the employment contract (for example frequent absenteeism, long-term absence, etc.)
Date of the beginning and, if possible, the end of the disability
Type of disability
Signature of the physician and the address of the physician's office
Distinction between short-term absences (a few days) and long-term absences (more than a few weeks) of employees
